Rights:
- You have the right to be treated with respect and for your dignity to be maintained during consultations.
- You have a right to request to see a GP/Clinician of your choice, please note this may not always be possible at the time of the appointment request.
Responsibilities:
- To treat all staff with respect.
- To keep appointments or to let us know in good time if you are not able to attend, in order for the appointment to be offered to another patient.
- You are responsible for ordering your medication before it runs out. 48 hours notice is required for prescription requests. It may not be possible to issue prescriptions for collection on the same day.
- To tell us immediately if you change your address or telephone number.
- You have a responsibility to treat the GP, and all staff with respect. Violence, aggression and abuse by patients will not be tolerated. The Practice operates a Zero Tolerance Policy. Patients who are violent, aggressive or abusive to the GP, staff or any other individual within the practice will be removed from the practice list.
